Cartogram Inc and its members, subsidiaries and affiliates (hereinafter “Cartogram”) values your privacy and the confidentiality of the information you choose to share. This statement pertains only to data collected via the Cartogram website (“Site”) and the Cartogram mobile and web application (“App”)

WHAT INFORMATION DOES CARTOGRAM COLLECT?

When you use the Site or the App, Cartogram may collect certain personally identifiable information (“Personal Information”) including the following:

Name
Phone Number (if calling into our call center)
Any information entered or submitted on contact forms at www.cartogram.com
Information provided in the Epic GetFutureAppointments API. Cartogram does not control the data returned by this API and such information may change over time.
User ID from Third-Party Apps

Examples include Epic, Cerner, and others

Future Appointment Information provided by third parties
Precise geographic location

In addition, when you use the Site or App, the following types of information may all be logged automatically (“Usage Information”):

Type of mobile device you use
Operating system of your mobile device
Wireless carrier
Number of visitors and page views
IP Address
Website pages visited prior to visiting the Site or App
Browser type
Search queries
Routing information
Technical information about your device, system and App software, and peripherals

HOW DOES CARTOGRAM USE YOUR INFORMATION?

Cartogram may use your Personal Information to help you locate the location of a future appointment or destination location and provide you directions to that appointment or location. Cartogram may use your Personal Information to locate your current location on a map to help you find your way throughout a facility or campus.

HOW DO WE USE PRECISE LOCATION INFORMATION?

Cartogram may use your precise geographical location information in order to provide you with information and otherwise provide services and functionality that you have requested. You may choose not to have your location disclosed when prompted upon accessing the Cartogram website or mobile application, but functionality may not be possible without such information. An anonymous location may be provided to Google within an API request in order to provide routing information. No personal information is provided to Google in this request.

HOW DO WE DISCLOSE YOUR INFORMATION?

Cartogram does not disclose any of your information with third parties other than as follows: (i) to the extent necessary to comply with applicable law and our legal obligations, such as to comply with a subpoena or similar legal process; (ii) when Cartogram believes in good faith that disclosure is necessary to protect its rights, your safety or the safety of others (iii) with third party vendors, consultants and other service providers who work for us and need access to your information to do that work; (iv) to investigate fraud, or respond to a government request; (v) in connection with or during negotiation of any merger, financing, acquisition, or dissolution, transaction or proceeding involving sale, transfer, divestiture, or disclosure of all or a portion of our business or assets to another company. Cartogram does not access your contact information or address book within your Device.

Cartogram may also share aggregated or de-identified information for any reason and at any time.

HOW DO YOU OPT-OUT?

You may opt-out of providing further information to Cartogram at any time by simply discontinuing use of the web app, or by uninstalling the mobile app, or a Cartogram-enabled mobile app using the standard uninstall processes available on your mobile device or via the applicable Application marketplace or store. You may opt-out of providing precise geographic location information by choosing not to allow tracking when asked. Opting out of certain types of information collection may limit your ability to use certain features or functionality within the App.

WHAT ABOUT THIRD PARTY TRACKING OR ADVERTISING?

Sending email directly to an email address found on a Cartogram website is not secure. Unsecured information transferred over the Internet is similar to a postcard being sent through the mail. It is recommended that you do not send sensitive personal health information to Cartogram or anyone else using unsecured means. Should you choose to supply confidential information in this manner, you do so at your own risk.

If you visit the Cartogram website at www.cartogram.com, Cartogram may use your information to re-target or re-market to you via advertisements. Cartogram partners with a third party ad network to manage our advertising on other sites. Our ad network partner uses cookies and Web beacons to collect non-personally identifiable information about your activities on the Site and to then provide you with Cartogram advertising on other websites. We may also place a persistent third-party cookie (provided by Google) on your hard drive if you visit www.cartogram.com. If you wish to not have this information used for the purpose of serving you targeted ads, you may opt-out by utilizing the Google opt-out tool. Please note this does not opt you out of being served advertising. You may continue to receive generic non-targeted ads featuring Cartogram.

WHAT ABOUT CHILDREN?

Cartogram’s websites and mobile application are intended for adults and young adults age 13 and over, and they are not intended for persons under the age of 13. Any child under the age of 13 should not register to use any portion of the Cartogram website and/or mobile application, or provide any personally identifiable information. It is not Cartogram’s desire or intent through its website and/or mobile application to collect and store any personally identifiable information from any person Cartogram actually knows to be under the age of 13. If Cartogram is made aware of collecting information of a child under the age of 13, Cartogram will delete this information.

WHAT ABOUT SECURITY?

Cartogram takes commercially reasonable steps to help protect your information against loss, misuse and unauthorized access, or disclosure. HOWEVER, CARTOGRAM EXPRESSLY DISCLAIMS ANY REPRESENTATION OR WARRANTY, WHETHER EXPRESS OR IMPLIED, WITH RESPECT TO ENSURING, GUARANTEEING OR OTHERWISE OFFERING ANY DEFINITIVE PROMISE OF SECURITY IN CONNECTION WITH YOUR INFORMATION.

WHAT ARE YOU CONSENTING TO?

By using the Site or App, you are consenting to the use and processing of your information as set forth in this Privacy Notice, all of which activities will take place in the United States. If you reside outside the U.S., your information will be transferred to the U.S., and processed and stored there under U.S. privacy standards. By using the Site or App and providing information to us, you consent to such transfer to, and processing in, the U.S.

Third parties

Cartogram.com uses WPMU DEV, a third-party cloud storage tool, to store backups of its audit logs where personal information is collected.

Additional data

This site creates and stores an activity log that capture the IP address, username, email address and tracks user activity (like when a user makes a comment). Information will be stored locally for 30 days and remotely for 1 year. Information on remote logs cannot be cleared for security purposes.
